Recovery burden after biopsy
(Agent report, 2026-09-03. Reviewed by Kya at synthesis.)
Q1 Recovery burden — biopsy-specific series (small)
- Zhang 2023 World Neurosurg (PMID 37004883): 11 endoscopic biopsies of undifferentiated CS lesions; diagnosis 10/11; "no significant difference in prebiopsy and postbiopsy SNOT-22"; 1 epistaxis needing return to theatre; no deaths; 6/11 got non-RT treatment.
- Liu & Di 2009 (PMID 19452412): 10 CS biopsies, 100% dx; BUT 1 ICA injury (balloon occlusion), 1 transient CSF leak.
- Kelley 1999 (PMID 10088024): 7 sphenoid/clival; 5/6 dx; 1 CSF leak, 1 intra-op bleed controlled; no deaths.
- Kang 2022 Pituitary (PMID 34471994): 39 endonasal biopsies, median 125 min, no CSF leak/infection.
- Percutaneous foramen ovale alternative: Dellaretti 2017 SR 65/75 dx, 3 permanent deficits, no mortality; Messerer 2012 86% productive, sens 0.83 spec 1.0.
- Upper-bound recovery (full pituitary resections): ambulatory protocol Sci Rep 2020 (PMC7297807) median surgery 82 min, 18/20 home same day, LOS 31h, no readmissions. Thomas 2014: 92% discharged POD1, LOS 1.16 d. FESS pain VAS 1.2 day 0, 1.05 day 1 (PMID 37250667). ADL return (PMC10009828): median walking 1 d, shower 5 d, >97% basic ADLs within 1 month; nose blowing 44 d; lifting >10kg 59 d; non-physical work 32 d.
Q2 QoL trajectory
- Little 2015 (n=218 multicentre): nadir at 2 wk, back to baseline at 3 months. Wu 2018: same. McCoul 2012: "predictable and self-limited". PMC10724315: 89% found nasal morbidity acceptable.
Q3 Biopsy and later surgery / seeding
- Stacchiotti 2015 (snippet): pre-op biopsy "can be avoided in selected cases of suspected skull base chordoma if reaching the tumour site would be problematic or when the risk of unrecoverable seeding is felt to be high"; "biopsy track should be included in the surgical resection". Default = pre-op histology; skull base = permitted exception.
- Fernandes Cabral/Gardner 2018: 2/173 EES clival chordoma seeding (1.15%), both after RESECTION.
- PMC4899426 review: 1.3-7.3% seeding after resection in 5 largest series (mean 2.7%, 930 pts); all 34 cases after resection. No skull-base seeding attributed to diagnostic biopsy alone found.
- Endonasal biopsy corridor = same corridor later resection removes.
Q4 Misdiagnosis on imaging alone
- Rosenberg 1999 (200 SB chondrosarcomas): 37% arrived labelled chordoma; chondrosarcoma 10-yr DSS 99% vs chordoma ~35%.
- Clival mets mimicking chordoma (PMC3774822): 47 cases, 36% first presentation of unknown primary; MRI "indistinguishable".
- Karras 2016: presumed chordoma encasing both carotids = prolactinoma (tablets). Li 2023: 5.8cm clival mass = ectopic ACTH adenoma. Li 2025: chordoid meningioma vs chordoma only by brachyury.
- PMC9944302 (42 clival tumours endoscopic centre): chordoma 38%, mets 19%, chondrosarcoma 12%, plasmacytoma 2.4%…
Q5 Does treatment relieve pain/neuropathy?
- Fatemi/Kelly 2008: 80% cranial neuropathy improved/resolved after endonasal chordoma removal; 67% V, 75% VI.
- PMC9944302: 67% pts / 75% CN palsy improved after surgery.
- Hantelius 2024 (prospective, 101 TSS): disabling headache MIDAS 60->10, headache days 45->6 per 90d, EQ-VAS 50->80.
- Tumour-related TN meta-analysis 2025 (PMC12048969): adequate relief 73% after SRS; MS 91% for petroclival meningioma.
- Mayo 2022: new CN deficits 32% pre-2013 -> 2.6% after (modern endoscopic).
Q6 GA risk healthy 33yo
- NAP7 2023: RCoA 1 in 100,000 anaesthetic death healthy adult; elective day case cardiac arrest ~1/1600, 0 deaths in 10,045; deaths <1/10,000 elective.
- Schiff 2014 BJA (1.37M ASA I-II): death/serious complication 26.2 per million; anaesthetic-attributable 7.3 per million.
Synthesis
Biopsy = ~1-2h GA, one night (or day case), pain ~1-3/10, nasal crusting 2-6 wk, basic activity within ~1 wk; anaesthetic risk negligible; real procedural risk = small single-digit % of bleed/CSF leak needing intervention (small old series each had 1 event in 7-11; carotid injury 1/10 in 2009 series). No evidence biopsy compromises later resection. Not having dx: 37% of chondrosarcomas mislabelled chordoma; mimics treated with tablets/steroids/chemo; management changed in >50% in CS biopsy series; no tumour-directed treatment (which is what relieves pain, 67-91%) can start without tissue. Deferring spends energy, doesn't save it.
